The main causes can be summarized into three key points:
- Cartilage shape and orientation: The "alar cartilage (lower lateral nasal cartilage)" that supports the nasal tip may be spread horizontally or excessively curved convexly. Additionally, "cephalic malposition" (where the cartilage is positioned higher than usual) is also a major factor in making the nasal tip appear round.
- Distance between cartilages: When the left and right alar cartilages are separated, the nasal tip may appear bifurcated or give a flat, wide impression.
- Soft tissues (subcutaneous fat and skin): The thickness of the skin itself, the SMAS layer (the underlying facial fascia), and the volume of fat are also important. In particular, Asians tend to have thick skin and weak cartilage, so the cartilage shape is not well reflected on the surface.

Specifically, techniques such as "transdomal suture" (narrowing the dome-shaped portion of the cartilage) or "interdomal suture" (drawing the left and right cartilages together) are employed. Since this technique does not destroy tissue, it is expected to suppress the risk of relapse while providing delicate definition (sharpness of contour) to the nasal tip.
2. Cephalic Trim
This technique involves removing part of the upper (cephalic) portion of the alar cartilage to reduce the volume of the nasal tip. Additionally, onlay grafts and other techniques that overlay cartilage on the nasal tip are considered effective for achieving sharpness that can compete with thick skin at the nasal tip. For patients with thick skin, since merely refining the cartilage shape may produce little change, these grafting procedures are often combined.
