Facial redness and post-inflammatory erythema (Erythema) are not merely superficial skin problems; they involve complex vascular changes and inflammatory reactions occurring deep within the skin. Medical research has demonstrated that red skin conditions and pigmentary disorders are deeply associated with increased vascular distribution in the skin and elevated levels of a protein called vascular endothelial growth factor (VEGF) (Iranmanesh et al. 2021). As this VEGF increases, excessive new blood vessels are formed beneath the skin, or existing vessels dilate, resulting in a red appearance visible through the skin's surface.

Furthermore, in this study, confocal microscopy-based image analysis was performed as an objective evaluation, and it was statistically significantly shown (p < .001) that facial pore areadecreased by 58.7%after treatment (Cho et al. 2012). Additionally, in another study (Min et al. 2015), comparing treatment of each half of the face with FMR versus conventional bipolar RF, FMRreduced inflammatory acne lesions by 80%and non-inflammatory lesions by65%, and was also shown to be highly effective against acne-related post-inflammatory erythema (Min et al. 2016). "Inserting needles and delivering electrical current"—hearing only this explanation would make anyone nervous. But when you look at the data, the picture changes.
According to a systematic review and meta-analysis (Foppiani et al. 2025) covering 21 studies involving 540 patients, microneedling treatment has been proven to be an extremely safe option for facial rejuvenation. The most commonly reported side effect was "transient erythema (temporary redness)," with an occurrence rate of only6.77%(Foppiani et al. 2025).
Other side effects have even lower percentages. Petechiae and edema (swelling) occurred in 2.07% of cases, scaling (skin peeling) in 1.65%, burning sensation in 1.5%, dry skin in 0.55%, and temporary post-inflammatory hyperpigmentation or prolonged needle marks in only 0.5%, while pruritus (itching) was reported in 0.41% (Foppiani et al. 2025). Additionally, another systematic review (Sitohang et al. 2021) analyzing 9 randomized controlled trials (involving a total of 341 participants) confirmed that microneedling has significantly shorter downtime compared to chemical peeling and ablative laser treatment, with no serious adverse events reported (Sitohang et al. 2021).
Pain and duration of erythema: Real patient experience
"I understand the numbers. But how much does it actually hurt?"—data answers that question too.
In the aforementioned study by Cho et al. (2012), regarding pain during FRM treatment, 60% of patients rated it as "mild"、20% answered "no pain"and in total,80% of patients found the pain during treatment to be within an acceptable range. Interestingly, while this study found no statistically significant changes in transepidermal water loss (TEWL) or sebum measurements used to assess skin barrier function, skin surface roughness temporarily became slightly rougher 2–4 weeks after treatment, thenshowed marked smoothness improvement by 8 weeks(Cho et al. 2012). Additionally, dermal density measured by ultrasound showed a significant increase by 8 weeks, objectively proving that the skin's deeper structure was being rebuilt (Cho et al. 2012). This reconstruction of collagen and elastin at the dermal level directly leads not only to reduced facial redness but also to tightened pores and improved skin firmness, creating high patient satisfaction.
