The Pitfall of "Cosmetic Surgery in Korea Can Keep Rhinoplasty Costs Down"
Rhinoplasty in Korea tends to have a cheaper surgical fee itself compared with Japan. Many people have probably seen information like "Korean cosmetic surgery has good cost-performance" on social media and elsewhere.
However,Deciding by the surface cost alone is dangerous. Traveling to Korea necessarily adds various expenses such as airfare, accommodation, local stay costs, and interpreter fees. Furthermore, you must consider the possibility of traveling multiple times for post-op suture removal and follow-up.
Rhinoplasty does not necessarily complete in one go. In one paper, a crooked nose (crooked's) revision—the reoperation rate is very high, according to such data. If a failure or complication occurs and revision surgery becomes necessary, you'll have to travel to Korea repeatedly.
The structure of the nose is very complex, requiring a balance of the various forces (deforming forces) acting on the bone and cartilage. In revision surgery, a "major operation" that completely releases these forces is required.
Also, if past surgery has used up the septal cartilage (the cartilage dividing the left and right of the nose), revision surgery requires newly harvesting rib cartilage (chest cartilage) or the like, which conceals the risk of being more expensive than having it done in Japan.
Different From Online Rumors? The Difference in "Technique" Between Korea and Japan in Rhinoplasty
In rhinoplasty, the technique of using artificial materials such as silicone implants to raise the Asian nasal bridge is widely used. At cosmetic-surgery clinics in Korea too, cases where silicone is chosen in pursuit of easy, dramatic change are not few.
It is sometimes said so in general, but actual research data suggests a different risk.
Among the complications ofaugmentation rhinoplasty(augmentation rhinoplasty: surgery to raise the nasal bridge)'s serious complication iscapsular contracturecan be cited. This is the phenomenon where the area around the implant hardens due to a foreign-body reaction in the body.
As capsular contracture progresses, the nasal skin is pulled, and it may turn into a "short nose" state where the nose turns up and deforms shorter. Once you fall into this state, the tissue of all layers, including the nasal skin and mucosa, is destroyed, and revision surgery to restore it becomes extremely difficult.
Therefore, in recent years, to avoid the risks of artificial materials, the "septal extensionseptal extension surgery," which grafts autologous tissue (one's own body's cartilage, etc.) to extend the nose, has attracted attention.
Decoding "the Risks and Failures of Having Rhinoplasty in Korea" From Research Data
To correct severe deformity, you need to reconstruct all three elements—skin, framework, and mucosa. To avoid artificial materials, rhinoplasty using rib cartilage, which can secure sufficient strength and quantity, is sometimes recommended. However, it's not that autologous tissue is absolutely safe.
Rib cartilage carries the risk of warping after surgery and the risk of calcifying with age (hardening and becoming hard to work with). A scar of about 5 cm may remain at the harvest site.
Also, there is a technique of finely crushing the cartilage and wrapping it in fascia. According to one paper's data, using this technique, a37.4% absorption rate at 18 months post-op (the grafted cartilage being absorbed by the body and decreasing)was reported.
Furthermore, there is also a report that shaving the nasal bone or cartilage excessively without performing appropriate reconstruction produces an unnatural "surgical-looking nose" in 25% of patients. In a foreign country with a language barrier, how accurately you can understand these specialized risks and possibilities of failure is put to the test.
Downtime and Aftercare Divide "Failure"
Rhinoplasty is not "complete once the surgery is over." Appropriate aftercare during the post-op recovery period greatly determines the final result.
In complex surgery such as correcting a deviated septum, a splint (fixation device) needs to be placed inside the nasal cavity until days 5–7 post-op. Also, the external tape fixation is sometimes recommended to continue for up to about 6 weeks to suppress swelling and make the skin adhere.
One study showed that in post-op evaluation, the NOSE score indicating nasal obstruction improved significantly from 62.1 pre-op to 9.2 at 2 months post-op. However, to obtain such good results, thorough post-op management is a prerequisite.
If an infection occurs after surgery, prompt response is essential. One paper found that, as complications, septal abscess (pus accumulation) occurred in 6.9% and wound dehiscence (the wound opening) in 3.4%.
In rhinoplasty in Korea, where there is physical distance, there is a risk that emergency administration of antibiotics or irrigation treatment is delayed. Not being able to respond immediately to minor post-op abnormalities may lead to irreversible failure.
