The double eyelid buried suture method is a procedure that forms a double eyelid line using a special medical suture without incising the eyelid. By threading the suture through the inner side of the eyelid skin and connecting the skin with the levator palpebrae superioris aponeurosis (or tarsal plate), a natural crease is created. Since it involves no incision, it leaves no scarring and features minimal downtime as its greatest advantage. Mechanism of the Procedure and Medical Basis

Double eyelids form when adhesion occurs between the eyelid skin and the levator palpebrae superioris aponeurosis or tarsal plate. The buried suture method realizes the double eyelid line by artificially creating this adhesion with sutures. According to a review of upper eyelid blepharoplasty published in the Indian Journal of Ophthalmology (2017), the eyelid region is a central aesthetic unit of the face and is considered a major target area in plastic surgery from both functional and cosmetic perspectives. This method places the knot on the back side of the eyelid (conjunctival side) without making any incision on the eyelid surface. Medically, it is called transconjunctival buried suture double eyelid surgery. With no indentation on the surface, it delivers natural results, and makeup is possible on the day of the procedure. Swelling is minimized, making it suitable for those who prioritize minimal downtime.

Zetith eyes Six Method (6-Point Fixation)

This method involves threading a single suture through 6 points in a cat's cradle pattern with only one knot. By creating six fine holes in the upper eyelid and connecting the skin and levator muscle to promote natural adhesion, the line is designed to be highly resistant to detachment. It accommodates both the levator method and tarsal plate method, and if suture removal is necessary, it can be performed from a single point. It is suitable for those with relatively thick eyelids or those who prioritize greater durability.

Procedure Flow

During consultation, discuss with the physician your desired double eyelid width, shape, and technique variation. On the day of the procedure, after cleansing, topical anesthetic eye drops and local anesthesia are administered to numb the eyelid before performing the procedure. The procedure takes approximately 20-30 minutes for both eyes. There may be a slight pinching sensation when the anesthetic is injected, but there is minimal pain during the procedure. After the procedure, we recommend cooling with ice packs to minimize swelling.

Downtime and Postoperative Course

Minor swelling and bruising may occur immediately after the procedure and into the following day. While individual differences vary, these typically subside naturally within about one week. With the Intact Method, makeup is possible on the day of the procedure, but with the Cross Method and Six Method, we recommend refraining from eye makeup for 48-72 hours. Contact lenses should be worn after the swelling has subsided (guideline: 3-5 days later). Avoid strenuous exercise and prolonged bathing for about one week. The double eyelid line takes time to stabilize, typically 1-3 months depending on the individual.

Safety and Complications

The buried suture method is a minimally invasive procedure without incision, so the risk falls into a relatively low category.
