What changes with liposuction equipment is whether there is a preliminary step to "loosen" the fat, and what that heat or vibration does to the skin and surrounding tissues. There are comparative trials showing that VASER, which uses ultrasound, produced greater skin contraction and less bleeding than conventional methods, but pain, swelling, and aesthetic satisfaction showed no difference at the 6-month mark[1]. The volume removed and final appearance are determined more by the surgeon's design and technique than by the equipment; the device is simply one tool.
What does each device do?
Conventional liposuction involves moving a thin tube (cannula) to scrape away fat. Power-assisted liposuction uses a machine to vibrate that tube in fine motions to aid fat removal and reduce physical strain on the surgeon[2]. Neither method includes a preliminary step to "loosen" the fat.
VASER is a third-generation device with improved design compared to earlier ultrasound machines using hollow tubes; it delivers ultrasound from a thin probe-like applicator to emulsify (soften and dissolve) fat before suctioning it out[1]. Laser uses light heat to dissolve fat. Textbooks explain that energy-based devices such as lasers and ultrasound reduce operative time, surgeon fatigue, recovery time, and postoperative pain[3].
What did comparative trials reveal between VASER and conventional methods?
There is one randomized trial comparing left and right sides of the same individuals. In a multicenter trial assigning the left and right sides of 20 subjects to conventional and VASER methods respectively, skin contraction per liter of aspirate was 15.4% versus 10.1% at 3 months and 17.0% versus 11.1% at 6 months (VASER side greater), and bleeding per 100 cc aspirated was 11.2 cc versus 14.0 cc (VASER side 26% less)[1].
Conversely, subjective assessments—pain, swelling, appearance, and patient and physician preference—showed no difference between sides at the 6-month mark[1]. A separate review that reexamined this trial expressed the skin contraction difference as 6% and bleeding difference as 3 cc per 100 cc, assessing the advantage as modest[4]. The same numbers can be interpreted differently depending on where emphasis is placed.
What about laser?
Studies have reported skin contraction, but lack long-term follow-up. In a 10-subject study cited by the aforementioned trial paper, laser-assisted treatment showed 54% greater skin contraction at 3 months compared to conventional methods, but long-term follow-up was not reported[1].
Devices combining laser with radiofrequency or ultrasound also exist. A review of laser liposuction cites synergistic effects of fat reduction and skin tightening as advantages of combination approaches, while noting that the technique becomes more complex and costs may increase as drawbacks[5].
What are the weaknesses of heat-based devices?
Heat-induced damage can result in an uneven contour. Textbooks note that heat-generating devices used within the body—such as lasers, ultrasound, and radiofrequency—affect not only the targeted fat but also unintended fat and the tissues supporting the skin, potentially causing heat-related contour deformities.[3].
Despite its widespread use, VASER lacked clear usage guidelines for a long time. In 2023, five U.S. plastic surgeons conducted three rounds of consensus discussions over four months and produced 18 consensus statements covering device settings, postoperative instructions, adverse effects and complications, and best practices.[6]The fact that consensus was necessary illustrates that settings and technique directly influence outcomes.
How much does recovery differ?
Recovery variation may be more dependent on postoperative care than on device differences. In a within-subject comparison study, there was no difference in subjective pain and swelling between sides.[1]Textbooks state that energy devices reduce recovery time and pain;[3]however, domestic guidelines specify that regardless of device, liposuction requires strict and ongoing compression therapy combined with self-care.[7].
Should you choose based on the device? Or the surgeon?
A device is a tool; how it is used is determined by the surgeon. A report of 736 high-definition abdominal contouring cases using VASER circumferentially explains that ultrasonic fat emulsification aims to facilitate aspiration, preserve blood flow, and improve long-term appearance.[8]A single-center study of VASER liposuction also notes that targeted action reduces burden on surrounding tissues and prevents damage to blood vessels, lymphatic vessels, and the fascial fibrous attachments to skin.[9].
These are explanations from the device user, and the device itself does not guarantee the final result. Even with the same VASER, settings, depth, and the volume and extent of aspiration differ by surgeon.[6]Rather than asking "which device," it is more useful to ask "what and how will you remove with that device" to get a realistic picture of the outcome.
What to confirm during consultation
Verify the reason for choosing that device for your specific area and skin condition, the settings and technique to avoid heat-induced damage, and the plan for postoperative compression and follow-up visits. In areas where skin Sagging (skin laxity) is significant, ask your surgeon directly so you do not base your expectations solely on the difference in skin contraction.
Skin thickness and fat distribution vary by area and individual, so which device is suitable will be determined during consultation after examining your body. You should also ask in advance about management of unevenness from over-removal.
